This system is directly linked to your patient record, which means:

• Your request is saved securely into your medical record

• Everything is traceable and auditable

• Requests reach the right team quickly, helping us to deal with them more efficiently

Please use Accurx for the following:

• Repeat prescription – order a prescription or ask a question about your medication

• Fit (sick) note – request a medical statement about your fitness to work

• Routine care appointment – such as long-term condition reviews, medication reviews, vaccinations and screening

• Test results – ask about the results of a recent test

• Referral follow-up – query an existing referral

• Doctor’s letter – including private, insurance, or educational letters

• Other admin requests

Important

We will no longer accept requests by email.

Exceptions

We understand that not all patients can use online forms. If you have a learning disability, are elderly, have significant mental health needs, or do not have access to a mobile device or the internet, please contact the practice directly and our reception team will help you.
